SUBSTANCE: group of inventions refers to medicine, namely to resuscitation, and can be used for prolonged artificial lung ventilation. To this end, a method is proposed comprising administering as an endotracheal tube with a cuff, a device that comprises an endotracheal tube with a cuff, a vent diffuser, tubes for air supply to the ventilation diffuser and cuff, a deflector in the form of a leaky elastic membrane, a tube for emergency air discharge. And the tube for air supply to the cuff is carried out along the inner surface of the endotracheal tube. Vent diffuser is located above the cuff, and the deflector does not reach 1–2 mm to the tracheal wall. Tube for emergency air discharge and aspiration from the respiratory tract above the cuff in the working position of the endotracheal tube is arranged with the possibility of locating in the initial part of the trachea. After the introduction of the device, in its cuff reduce the pressure by 30–50 %. Air diffuser is injected with air under pressure, which is 80 % of the pressure in the cuff, so that air through the diffuser openings enters the trachea above the cuff. Deflector in the form of a non-hermetic elastic membrane directs air upwards along the walls of the overlying airways, forming upward air currents above the cuff with the possibility of evacuating air through the natural airways. Air can also be evacuated through a pipe for emergency air discharge, through which it is also carried out aspiration. Evacuation of antiseptic solutions during the sanitation of the space above the cuff is also carried out through the emergency relief tube.
EFFECT: inventions provide prevention of infection of the lower respiratory tract and post-traumatic tracheal stenosis with prolonged artificial ventilation of the lungs due to a significant leveling of the effect of cuff pressure on the endotracheal tube on the mucous membrane of the trachea, and also provide active counteraction of the descending infection due to effective sanitation of the space above the cuff.
