SUBSTANCE: invention relates to medicine, namely to anesthesiology and surgery, and can be used for laparoscopic operations on the large intestine in high risk patients. To do this, a bolus of rocuronium is injected in a dose of 0.6 mg/kg followed by a constant infusion at rate of 0.3 mg/kg/h to maintain a neuromuscular block at a PTC (post-tetanic count) level < 5. Then, carboxyperitoneum is created, maintaining the level of intra-abdominal pressure no higher than 8 mm Hg. At the end of the operation, during extubation, the neuromuscular block is reversed by the administration of sugammadex in a dose of 4 mg/kg.

EFFECT: method provides adequate and gentle anesthesia in this category of patients, allowing the use of inhalational anesthetics in minimal concentrations with simultaneous achievement of a deep neuromuscular block, and also allows to maintain the optimal volume of the abdominal cavity for safe and comfortable operation with laparoscopic access while eliminating the negative effect of prolonged carboxiperitoneum.
