FIELD: medicine.
SUBSTANCE: invention refers to medicine, namely to dentistry, and is intended for use in treating patients with dento-facial abnormalities combined with a syndrome of pain dysfunction of the temporomandibular joint (TMJ). Clinical, functional and X-ray examinations of the patient are carried out using orthopantomography (OPTG), lateral teleradiography (TRG) with cephalometric analysis, cone-beam computed tomography (CBCT) of the TMJ in two positions: open and closed mouth, magnetic resonance tomography of temporomandibular joint, kinesiography, ultrasonic examinations of temporomandibular joint and diagnostics of somatic dysfunction at osteopath. Before the beginning of the treatment, the upper jaw splint is prescribed to eliminate the muscular imbalance for 2 weeks. Then splint is made - medical-diagnostic-occlusal tray on lower jaw from 37 to 47 teeth, and its wearing is prescribed for period from 6 to 12 months. In the same period, osteopathic treatment and elimination of somatic dysfunctions are carried out. After 6 months of treatment on the splint, again, to assess the correction of the position of the temporomandibular joint, an MRI examination is performed. Then orthodontic treatment of patient and preparation for orthognathic surgery, at the clinical stage, the dental arches are scanned with an intraoral scanner and an anthropometric analysis is carried out, the desired parameters of the bite of the patient after the operation are determined. Then, at the laboratory stage, a splint template is made. At the surgical stage, at least one or, depending on the patient's diagnosis, a combination of operations from the following row is performed: displacement of the upper jaw - LeFort I osteotomy; movement of the lower jaw - intercortical osteotomy of the lower jaw; displacement of the chin - an increase in its projection, reduction, displacement, shape correction; performing marginal resections of the lower jaw with creating the symmetry of the contours; displacement of zygomatic bones - projection enlargement. After the operation, a splint is fitted, and the microimplants are additionally inserted through the prepared holes, after which the elastic orthodontic elements are fixed on the head of the microimplant. At the post-surgical orthodontic stage, 1.5 months later, the following examinations are repeated: OPTG, TRG of head in lateral projection with cephalometry, CBCT of temporomandibular joint; in the retention period, a fixed retention apparatus is made on both jaws; additional examinations are carried out: OPTG, TRG of head in lateral projection with cephalometry, CBCT, ultrasound of temporomandibular joint and recording results of complex treatment of patient.
EFFECT: disclosed is a method of treating patients with occlusion anomalies combined with a syndrome of pain dysfunction of the temporomandibular joint.
