FIELD: medicine.
SUBSTANCE: invention relates to medicine, specifically to ophthalmology. Form the bed of the cornea of the recipient and the donor graft using a femtosecond laser with diameters equal to each other, performing a circular incision of the cornea of the recipient and the donor transplant concentric with the limbus with an upper cut diameter of 6–9 mm with a step of 0.25 mm to a depth of 1/2 the thickness of the cornea, the lower one – 5–8 mm with a step of 0.25 mm. Profile of the slice is formed by the type of "mushroom" with the formation of a "step" with an angle of 90 degrees, the diameter of the upper slice 1 mm larger than the diameter of the lower slice. Simultaneously with trepanation of the stepped bed of the cornea of the recipient, a lamellar cut is formed, which is a continuation of the “step” to the limb, in the form of a ring, concentric to the limb, the inner diameter of which is equal to the diameter of the upper cut, and the outer one exceeds it by 1 mm. Intrastromal ring with a diameter equal to the outer diameter of the lamellar cut is implanted into the lamellar cut formed in the peripheral part of the cornea of the recipient. For correction of myopia, an intrastromal ring with an external diameter of 0.5 mm more than the external diameter of the lamellar cut is used.
EFFECT: method improves visual function by reducing postoperative astigmatism after end-to-end or deep layered keratoplasty, and also prevents secondary ectasia.
