FIELD: medicine.
SUBSTANCE: invention relates to medicine, namely ophthalmosurgery. For the surgical treatment of overripe cataracts, an anterior capsule is opened, anterior capsulorhexis is performed, iris retractors (IR) are installed, lens mass phacoemulsification, intraocular lens implantation and IR removal are performed. During the formation of the anterior circular capsulorhexis, four irio-capsular retractors (ICR) are installed in series with an emphasis at the equator of the capsular bag through previously formed vertical paracentesis, why after opening the anterior capsule in sector 90° ICR are implanted into the resulting rupture with emphasis in the equator of the capsular bag. Then, the capsule is opened in the second ninety-degree sector with installation of the second ICR in this sector. Further, the third and fourth ninety degree sectors of capsulorhexis are sequentially performed in a similar manner with installation in each sector of the ICR. First ICR are removed from the capsular bag and the iris is fixed to them, expanding the pupil, and complete the capsulorhexis, performing it with a diameter of 5–5.5 mm. Then, the ICR are again placed in the capsular bag for fixation in this sector, the intracapsular ring is implanted, the lens mass is phacoemulsified, the intraocular lens is implanted and ICR is removed, then the viscoelastic is washed out and the main incision and paracentesis are hydrated.
EFFECT: method provides creation of optimal conditions for performing phacoemulsification of overripe cataract by preventing uncontrolled ruptures of the capsular bag and rupture of the zonular fibers of the cinnamon ligament during the execution of the anterior circular capsulorhexis.
