SUBSTANCE: invention refers to medicine, namely to ophthalmosurgery. Method of surgical management of cataracts with lens subluxation involves opening of anterior capsule, performing anterior capsulorhexis, hydrodissection, phacoemulsification of nucleus of lens with subsequent removal of crystalline mass, implantation of intraocular lens (IOL). When forming the anterior circular capsulorhexis, four to six iris-capsular retractors (ICR) are sequentially fixed with emphasis in the equator of the capsular sac through the preformed vertical paracentesis, for which anterior capsule is perforated in two or three points in an initial sector of anterior capsulorhexis. That is followed by linear opening of anterior capsule by perforation points, and ICR is placed in said rupture with thrust in equator of capsular sac, second, third and fourth ninety degree regimens of capsulorhexis are sequentially performed in a similar manner in every sector of the ICR. First ICR is withdrawn from the capsular sac and the iris is fixed to extend the pupil, and the capsulorhexis is completed to the required value. ICR is re-inserted into the capsular sac for fixation in the first sector. That is followed by the phacoemulsification of the nucleus of the lens.
EFFECT: use of the invention provides optimal conditions for the surgical management of cataracts in lens subluxation and reduces the risk of intra- and postoperative complications by reducing the load on the tendon of Zinn and stabilization of the capsular sac in the sagittal plane during mechanical exposure of the capsular sac to the instruments and irrigation-aspiration.
