METHOD OF EVERSION CAROTID ENDARTERECTOMY Russian patent published in 2019 - IPC A61B17/00 

FIELD: medicine.

SUBSTANCE: invention relates to medicine, specifically to vascular surgery. Oblique ICA is inclined at the level of a bulb, the ICA is dissected from the side adjoining the "carotid body" longitudinally upwards to the upper arteriotomy level on the CCA. CCA is incised along an anteriolateral wall to a level coinciding with the length of the arteriotomy on the ICA. It involves an eversion endarterectomy of ICA, CCA and external carotid arteries (ECA). Arterial lumen is restored by anastomosis between ICA and CCA. End angle ICA is recorded after the endarterectomy. Artery walls are raised at diametrically opposite points. That is followed by visual determination of excess arterial fragments protruding beyond outer contours of the distal ICA. Thereafter, its excess fragments are excised. Then two tweezers are lifted and diluted CCA walls in the area of arteriotomy in diametrically opposite points, visually determined, and then excised excessive fragments of artery protruding beyond outer contours of proximal CCA.

EFFECT: method enables higher clinical effectiveness in patients, reduced number of restenoses and occlusions of reconstructed carotid arteries, preserved initial anatomy of CCA bifurcation, eliminating hemodynamically significant blood flow disturbances in bifurcation CCA and bulb ICA, as well as prevent acute onset cerebral circulation ischemic type.
