METHOD FOR SURGICAL MANAGEMENT OF PARASELLAR LOCALIZATION TUMORS WITH SPROUTING INTO THE OPTIC CANAL Russian patent published in 2019 - IPC A61B17/00

SUBSTANCE: invention relates to medicine, namely to neurosurgery. For surgical treatment of parasellar localization tumors with sprouting into optic canal, thermal craniotomy with combined approach including extradural and intradural approach to tumor and optic nerve channel is performed. If the extradural approach is performed after the pre-formed blood-supported periosteal aponeurotic flap on the branches of the superficial temporal artery and the small wing of the wedge-shaped bone is removed, partially incised anterior inclined appendix and performing decompression of upper and lateral walls of optic nerve canal. Thereafter, the intradural approach involves an arched incision of the dura mater, removing a major portion of the tumor and continuing the intradural resection of the remaining portion of the optic nerve canal with a T-shaped dissection of the dura mater above the anterior inclined process, obtaining additional optic nerve decompression, removing residual tumor fragments in the optic canal. Pre-formed blood-supported periosteal aponeurotic flap is placed in area of defect of ethmoid bone, fixed to edges of defect for dura mater and additionally sealed with biological glue.

EFFECT: method provides preserving optic nerve function, performing skull base defect repair in a single surgical cycle and avoiding postoperative liquorrhea and inflammatory complications.
