FIELD: medicine.

SUBSTANCE: invention refers to medicine, namely to neurosurgery, and can be used for access to a base of skull in surgical treatment of patients with complex cerebral aneurysms and tumours of parasellar localization. Method involves formation of pterional and orbital flaps. When forming the orbital flap, the first cut is made through the supraorbital edge of the frontal bone along the medial edge of trepanation from outside to inside. Second cut is made in the area of the lateral wall of the orbit, at the same time starting 3 mm below the frontal-zygomatic suture, ending in the area of the lateral edge of the superior orbital fissure. Third notch is made, at the same time starting medially along the base of the anterior inclined process to the visual canal, then in the direction from the inside to the outside to the first notch, after which the formed bone flap is removed. During cutting of the orbital bone flap, the first fixation point of the anterior inclined process to the sphenoid bone is crossed – a lesser wing, thereafter, a second anterior inclined appendix fixation point – a roof of the visual canal – is resected, after which a third anterior inclined appendix fixation point is transected – an area of connection with a visual strut. Anterior inclined process is removed as a single unit. After the main surgical stage, the orbital roof and calvarium are reconstructed with fixation of bone flaps.

EFFECT: method provides improved results of surgical treatment due to addition of craniotomy with resection of anterior inclined process.
