FIELD: medicine.
SUBSTANCE: invention refers to medicine, namely to neurosurgery in surgical management of patients with anterior skull base with invasive growth and extracranial distribution. Split flap is formed from the convexital surface of the skull, the corresponding size of the anterior cranial fossa (ACF) defect. Slots are made on the sides of the bone flap. Subdurally artificial DM is placed in a defect of a dura mater (DM) of a cerebral base, artificial DM larger than a DM defect in size. Periosteal flap on the vascular pedicle formed earlier is laid on the ACF base. Splitted bone flap is laid over periosteal flap. Flap is fixed with slots in an ACF defect and enveloped with a periosteal graft. Sealing is performed by means of tachocomb and fibrin-thrombin adhesive. Thereafter, the DM defect is closed with using a flap pre-formed from a wide fascia of thigh.
EFFECT: method enables avoiding prolapse of brain substance into extensive postresectional defect and development of corresponding neurological and infectious complications, higher clinical effectiveness.
