SUBSTANCE: invention relates to medicine, namely to neurosurgery, and can be used to treat patients with surgical pathology of the spine and neurovascular structures of the spinal canal. Method of percutaneous video endoscopic intervention on the structures of the spinal canal is carried out by two-stage intervention with changing media for endoscopy. At the first stage, after the working cannula with the endoscope is inserted into the epidural space, neural-vascular structures are dissected under conditions of irrigation with saline sodium chloride. After creating the operating space for manipulations stop the irrigation of saline. Fill the operating space with air coming from the outside through the irrigation and manipulation channels of the endoscope along the gradient – into the low pressure area due to the vacuum effect achieved by continuously pumping air from the epidural space through the aspiration channel of the endoscope.

EFFECT: method reduces the risks associated with exposure of the neurovascular structures of the spinal canal to an overpressure/volume of liquid or gas, without reducing the image quality due to the fact that the endoscopy media change along the gradient to the low pressure area.
