METHOD OF ACCESSING AND CLOSING ACCESS TO A SKELETON OF A NASAL SEPTUM WITH ENDOSCOPIC SEPTOPLASTY Russian patent published in 2019 - IPC A61B17/24

Abstract RU 2699922 C1

FIELD: medicine.

SUBSTANCE: invention refers to medicine, namely to plastic surgery, and can be used in endoscopic septoplasty. Mucous membrane is incised on the side of the curvature and in front of it with transition to the bottom of the nasal cavity. One performs separation of the mucoperichondral, mucoperiosteal flap on the side of the incision. Lateral displacement of the mucoperichondral flap is performed. Free edge of flap after separation is sutured by one vertical turn of needle with absorbable material Vicryl or Polydixanone. Two points of fixation of the suture are formed at the edge of the flap. Both ends of the filament are taken into a clamp at distance of 8–10 mm from the edge of the flap. Clamp retaining suture ends is folded on patient's cheek on access side so that it freely hangs by pulling thread. Flap edges are shifted laterally from the septum, opening access to the nasal septum of the skeleton. That is followed by correction of the curved nasal septum. Thereafter, one vertical suturing of the nasal septum is performed proximally from the incision in the opposite direction. When the suture is tied, the edge of the incision is compared, and the mucoperichondral flap is pressed against the skeleton of the nasal septum.

EFFECT: method enables carrying out endoscopic septoplasty, monitoring the state of the common nasal passage, simplifying the surgical intervention and shortening the length of the operation by good endoscopic examination of the skeletal nasal skeleton, preserving the mucoperichondral flap, the technique of closing the approach and closing the wound.
