METHOD OF COSTAL CARTILAGE RESECTION Russian patent published in 2019 - IPC A61B17/00

Abstract RU 2702891 C1

FIELD: medicine.

SUBSTANCE: invention refers to medicine, namely to thoracic surgery and traumatology-orthopedics, and is applicable for mobilization of a sternocostal complex in any forms of chest deformation, as well as other diseases or conditions requiring resection of costal cartilages. Anterior chest wall is examined by determining the shape and size of the cartilage subject to resection. At the first stage, the perichondrium is dissected longitudinally and a guide channel is formed at depth of 2–3 mm, with width of 1 to 2 mm using a cutter with a protective sleeve. At the second stage in the proximal or distal part of the formed guide channel by drilling is immersed to the protective coupling located on the connecting neck at distance of 1.5–2 mm from the cutting head of the cutter, the cutter selected according to the size and shape of the cartilage and the main mass of the central zone of the costal cartilage, pushing the cutter along the cartilage. Cutter head shape is determined by shape of cartilage, and its diameter – by subtraction of minimum diameter of one end of costal cartilage from minimum diameter of other end. If observing a similar shape of the costal cartilage throughout its length, resection is complemented by a third step, for which the central area of the costal cartilage is treated with a cutter, diameter of cutting head of which is less than diameter of cartilage by at least 1.5–2 mm, immersing it in cartilage to protective sleeve located on connecting neck at distance of 1.5–2 mm from cutting head of cutter. Main mass of the central zone of the costal cartilage is removed directly or at an inclination of up to 45 degrees.

EFFECT: method enables providing resection of costal cartilage taking into account its morphological zones, namely resection of the central cartilage zone without damaging the perichondrium and the subperichondral zone, which in turn does not cause cartilage apposition growth disturbance.
