FIELD: medicine.
SUBSTANCE: invention refers to medicine, namely to neurosurgery, and can be used for surgical management of malignant ischemic stroke in a middle cerebral artery (MCA) basin. A skin incision is performed. Frontal temporomandibular parietal fragment of skull is resected. Resection of the frontal temporomandibular parietal fragment of the skull along its lower border is performed to a spinous opening. A dura mater is opened. A fragment of the labia of the cerebellum surrounding the lateral surface of the midbrain is formed. For this purpose, on the side of the lesion, deviating from the medial end of the upper stony sine by 0.5–1.0 cm backwards, the portion of the cerebellum nest adjacent to its anterior free edge is dissected in the form of a semicircle with diameter of 2–2.5 cm. cerebellar nasal fragment and anterior portion of para-hippocampal gyration of temporal lobe of brain are removed. That is followed by performing plastic surgery.
EFFECT: method enables highly effective surgical management of malignant ischemic stroke in the MCA pool and eliminates postoperative lethal cases by decompressing the temporal lobe and eliminating cerebral compression.
