FIELD: medicine, neurosurgery in particular.
SUBSTANCE: invention relates to medicine, namely to neurosurgery. Skin incision, resection of the frontal-temporal-parietal fragment of the skull are performed. An autopsy of the dura mater (DM) is performed. Encephalotomy is performed on the lateral surface of the temporal lobe on the side of the lesion, retreating 4 cm from the pole of the temporal lobe. Resection of the anterior temporal lobe is performed to the level of the temporal horn of the lateral ventricle. A fragment of the cerebellar tentorium surrounding the lateral surface of the midbrain is formed. The formed fragment of the cerebellar tentorium and the anterior part of the parahippocampal gyrus of the temporal lobe of the brain are removed, after which DM plastic surgery is performed.
EFFECT: method makes it possible to increase effectiveness of surgical treatment of malignant ischemic stroke in the medial cerebral artery basin, reduce the number of deaths in the postoperative period and ensure early restoration of the functional status of the patient.
