METHOD OF FLOW PREDICTION AND SELECTION OF A METHOD OF TREATING MUSCULAR DYSFUNCTIONS OF TEMPOROMANDIBULAR JOINT Russian patent published in 2019 - IPC A61B5/488 A61H39/00 A61H7/00

Abstract RU 2705240 C1

FIELD: medicine.

SUBSTANCE: invention refers to medicine, namely to surgical dentistry and maxillofacial surgery, and can be used in predicting clinical course and selecting a method of treating muscular dysfunctions of temporomandibular joint. That is ensured by electromyographic examination of the pterygoid muscles and oral cavity muscles of the right and left halves of the face at rest. Test is repeated three times every 10 minutes. Maximum and average amplitudes of arbitrary bioelectric activity (BEA) are recorded. If observing BEA values at rest within range of 0 to 90 mcV, physiologic muscle function restoration and trigger voltage suppression are predicted by 15-day course of therapeutic massage, kinesiotherapy and independent exercise. If observing BEA values at rest from 90 to 120 mcV, recovery of physiological muscle function is predicted and trigger voltage is relieved by 15-day course of therapeutic massage, kinesiotherapy, independent performance of the exercises together with application of preparation mydocalm for 30 days for 50 mg 2-3 times/day for first 10 days, then for 150 mg, 2-3 times/day – 20 days. If observing BEA values at rest above 120 mcV, partial recovery of muscle physiological function is predicted, trigger voltages are reduced by 15-day course of therapeutic massage, kinesiotherapy, independent performance of the exercises together with application of preparation mydocalm for 30 days for 50 mg 2-3 times/day for first 10 days, then 150 mg, 2-3 times/day – 20 days, followed by introduction of botulinum toxin single 250 units into the trigger region voltages. Facial and chin massage is performed, which consists of strokes, grinding, kneading and vibration. That is followed by a kinesiotherapy consisting of post-isometric relaxation of medial, lateral pterygoid muscles and oral cavity muscles. Then the patient independently performs the exercise: the mouth is half-open, the patient achieves complete relaxation of the oral cavity muscles for 30 seconds, then the upper and lower lips are pulled in by the patient, trying to bring them behind the dentures, mouth half-open, lower jaw shifted forward, exercise performed 2-3 times day for 30 seconds.

EFFECT: method enables removing trigger voltages of the musculo-articular complex, eliminating pain syndrome, conducting a differentiated treatment taking into account the actual condition of the musculo-articular complex, as well as selecting exercises to patients to rule out exacerbations of the disease.
