METHOD FOR ASSESSING THE EFFECTIVENESS OF THE NEOADJUVANT CHEMORADIOTHERAPY IN PATIENTS SUFFERING RECTAL CANCER Russian patent published in 2019 - IPC A61B6/00 A61N5/00 G01N23/00

Abstract RU 2705257 C2

FIELD: medicine.

SUBSTANCE: invention refers to medicine and can be used to assess the oncological patients treatment outcome using the neoadjuvant chemoradiotherapy (NCRT) patients with rectal cancer. For texture analysis, MP images are used in T2-VI mode in axial plane at the level of tumor center, obtained by means of pulse sequence FSE and with high spatial resolution: TR (repetition period of sequence) is 4,020 ms, TE (time of appearance of echo signal) is 97 ms, thickness of cut/pitch is 3.0/0.3 mm, FoV (size of field of view) is 250 mm, MTX (size of matrix/pixels) is 286×512. Further, selected images are loaded into Mazda ver.4.6 program and image segmentation is performed. Texture parameters are determined by matrix of co-occurrence of gray GLCM levels at distance of 2 pixels and in four different directions – 00,450,900,1350. Obtained values are averaged and 11 parameters of texture analysis are automatically calculated using a computer program. Of these, 5 texture analysis parameters are evaluated on the basis of a point system, specifically: AngScMom≥0.0022, then 1 point, if <0.0022 – 0 points; InvDfMom≥0.12 – 1 point, if <0.12 – 0 points; Entropy≤2.75 – 1 point, if >2.75 – 0 points; DifEntrpy≤1.32 – 1 point, if >1.32 – 0 points; SumEntrp≤1.8 – 1 point, if >1.8 – 0 points. And if total score >2, patient answered NCRT, if ≤2, the patient did not respond to NCRT.

EFFECT: method enables individualising a further therapeutic approach, using less aggressive, organ-preserving versions of the surgical management ensured by an integrated clinical evaluation of the therapeutic outcome involving the MRI data.
