FIELD: medicine.

SUBSTANCE: invention relates to surgery, in particular to cardiosurgery. What is presented is a method for combined myocardial protection in heart operations in newborns and infants by staged insertion of cardioplegic solutions into a coronary bed. Immediately before the operation, three cardioplegics are prepared from the official preparations, which are then introduced into the coronary bed in 3 stages; wherein at stage 1 (induction into cardioplegia), a solution is introduced into the aortic root with a syringe until the moment of asystole, containing: mannitol 12 mg/ml, magnesium sulphate 8 mg/ml, sodium bicarbonate 5.6 mg/ml, lidocaine 0.48 mg/ml, potassium chloride 2.8 mg/ml in amount of 10 ml/kg with a temperature of +4 °C; then at stage 2 (maintaining induction by blood cardioplegia), a solution containing sodium bicarbonate 2.6 mg/ml, potassium chloride 3.52 mg/ml, mixed with oxygenated blood at ratio of 1:4, a session duration of 2 minutes with temperature of the final solution +4 °C; at stage 3 (maintenance of cardioplegia), a solution with sodium bicarbonate content 2.5 mg/ml, potassium chloride 2 mg/ml is introduced, wherein solution supply rate is 5 % of the main flow rate, solution temperature is +4 °C, length of cardioplegia session 1 min. Blood/crystalloid ratio is 4:1, and the cardioplegia session is performed every 20 minutes. Method is realized using a device which includes a heat exchanger, an oxygenator and a vessel with a crystalloid cardioplegic solution, connected by means of lines for supply to a heat exchanger, a roller pump, devices for recording temperature and pressure.

EFFECT: technical result is providing adequate myocardial protection in surgical interventions in newborns and infants, leveling metabolic and electrolyte disorders when performing cardioplegia.
