FIELD: medicine.

SUBSTANCE: invention relates to the field of medicinal preparations containing inorganic active ingredients, used in cardiosurgery in the treatment of heart diseases with mitral valve involvement. For electromechanical arrest of the myocardium, a crystalloid component of del Nido cardioplegia is used, which is obtained from the following ratio of components: basic solution—1 l, Mannitol 15%—21.7 ml, magnesium sulphate 25%—8 ml, sodium bicarbonate 5%—21.8 ml, potassium chloride 7.5%—34.6 ml, lidocaine 2%—6.5 ml, pH value is 6.5–8.0, which is mixed with blood from an oxygenator in ratio of 4:1, using a standard double-circuit set for supplying cardioplegia with 1/4 inch lines, where the temperature control device is connected with cooling supply of 4–6 °C, the system is filled with a prime from oxygenator and 0.9% sodium chloride solution, then step-by-step pressure calibration of pumps with 1/4 inch lines is carried out. After the onset of the cardiopulmonary bypass, 3–4 minutes later, the ratio of the roller pumps with 1/4-inch lines of the crystalloid component/blood is set to 4:1 and supplied through the set for the supply of cardioplegia, cooled to 4 °C, into the pericardial cavity; after the appearance of the blood-stained solution, the main line is connected to the cardioplegic cannula installed in the ascending aorta, infusion into coronary vessels is carried out under pressure of 50–120 mm Hg, with a delivery rate of 200–300 ml, in amount of 20 ml/kg; after termination, recirculation of cardioplegia from heat exchanger into bag with crystalloid component of cardioplegic solution by del Nido is started.

EFFECT: method enables to provide the mixed solution and blood supply through the closed system, without disturbing the composition of the supplied solution, and introduction of an alternative method of myocardial protection using standard sets into the routine practice of a perfusionist.
