METHOD FOR STENTING OF BILE DUCTS BY MEANS OF COATED NITINOL SELF-EXPANDING BILIARY STENT Russian patent published in 2020 - IPC A61B17/00 A61F2/04

Abstract RU 2722880 C1

FIELD: medicine.

SUBSTANCE: invention refers to medicine, namely to abdominal surgery, operative endoscopy, and can be used for bile duct stenting with the help of coated nitinol self-expanding biliary stent. Cholangiostomic drain tube is inserted. Cholangiography is performed to calculate required length and diameter of stent, as well as performing its preliminary positioning and stent installation, stenting of common hepatic and bile ducts. After preliminary positioning of said stent, perforations are formed in cells of distal portion of stent and proximal to middle of stent, or in cells only of distal portion of stent, or in cells only of middle of stent, or in cells only proximal to the middle of the stent, or in the cells of the distal portion of the stent and its middle, or in the cells, proximal to the middle of the stent and its middle, or in the cells of the distal portion of the stent, as well as in the proximal direction of the middle of the stent and in the middle of the stent. It is perforated along the entire circumference for at least 0.5 cm with the subsequent installation of the perforated stent so that its non-perforated part overlaps the area of the stricture. Perforated portion of the stent is placed in the zone of the confluence of the cystic duct into a common hepatic duct and the region of integrating the segmental branches into the lobar bile duct, or only in the zone of the confluence of the cystic duct into the common hepatic, or only in the zone of confluence of the common bile and main pancreatic ducts, or only into the zone of segmental branches joining into the lobar bile duct or into the zone of confluence of the main pancreatic and common bile ducts and in the region of integrating segmental branches into the lobar bile duct, or into the main pancreatic and common bile duct confluence zone and into the segmented branches joining the lobar bile duct and the cystic duct into the common hepatic duct, or into a zone of confluence of common bile and main pancreatic ducts and into an area of cystic duct inflow into a common hepatic, or in a zone of confluence of common bile and main pancreatic ducts and a zone of integrating segmental ducts into a lobar bile duct.

EFFECT: method provides the possibility of stenting in the absence of an additional Wirsung's duct in the case of simultaneous inflow of the main pancreatic and common bile ducts, with functioning gall bladder and lobes and segmental ducts ensured by the perforated probe installation.
