FIELD: medicine.
SUBSTANCE: group of inventions refers to medicine, namely to ophthalmology, and aims at treating peripheral corneal ectasia. Intrastromal ring tunnel is formed by femtosecond laser and donor tissue is implanted therein. According to the first version, the tunnel is formed at depth of 80 % of the minimum thickness of the cornea with an external diameter of 9.0 mm and an inner diameter of 3.0 mm. Tunnel formation is completed by performing radial incisions of the cornea in the strong meridian crossing the tunnel. Then mechanically, radial incisions are followed by corneal dissection through the tunnel and thinning area toward the limb, thus forming an intra-stromal pocket within the cornea thinning zone. Created pocket is implanted with a refractive donor's lenticule obtained from the other patient in a SMILE procedure for myopia, pre-colored and cut off at the poles or a segment of the ring of donor tissue produced by means of a femtosecond laser or cut out manually. Thereafter, one intrastromal corneal segment is implanted in the tunnel with an arc length of 80 to 359 degrees, or two intrastromal corneal segments with an arc length varying from 80 to 160 degrees each, placing them in the tunnel diametrically opposite to each other. According to the second version, the tunnel is formed at depth of 80 % of the minimum thickness of the cornea, with an external diameter of 9.0 mm and an inner diameter of 3.0 mm. formation of the tunnel is completed by performing radial incisions of the cornea in the strong meridian crossing the tunnel. Refractive donor lenticule, obtained from the other patient in SMILE procedure for myopia, pre-colored and cut off at the poles or segment of the ring of donor tissue, produced by means of a femtosecond laser or cut out manually, is implanted into the tunnel into the thinning area. Thereafter, one intrastromal corneal segment is implanted into the tunnel under the lenticule or donor tissue with arc length varying from 80 to 359 degrees, or two intrastromal corneal segments with arc length varying from 80 to 160 degrees, diametrically opposite to each other. At that, as per the first version, and according to the second version the stages of operations are completed without suturing or with application of single corneal sutures on radial incisions. After 3–6 months, cross-linking of the cornea is performed.
EFFECT: group of inventions makes it possible to increase accuracy and predictability of refractive result and, as a result, to improve visual functions of patients, to stop the progressing of corneal ectasia, and also to increase availability of donor material.
