FIELD: medicine.
SUBSTANCE: invention refers to dentistry and can be used as an anesthesia aid in mandibular anesthesia accompanying dental interventions. Method of mandibular anesthesia is characterized by using a device comprising an arched body with extraoral and intra-oral parts. Extraoral part is made in the form of plate-like arc with end section with groove, on external bend of housing fixed ring-shaped grip for finger is installed. Intra-oral portion comprises a measuring rod with a graduated scale and a holder, wherein the end portion of the rod is in the form of a saddle, wherein the rod is movably installed along the through guide channel in the housing and is retained by means of the screw retainer, and additionally is equipped with an elastic element resting between the end seat of the rod and the housing. Anesthetic is fixed by fixing the end section of the arc with the extraoral part groove at the posterior edge of the mandible branch within the maximum concavity, after which the graduated rod is placed in the oral cavity, pulling back behind the holder. Rod end saddle is placed on the front edge of the mandible branch in the area of maximum concavity thus fixing the rod in the preset position by means of a fixator. Then, the device is removed from the oral cavity, the mandibular branch width is recorded, by which the optimal depth of the needle immersion is determined by calculation, which value is established on a line of the device for mandibular anesthesia by means of a movable limiter, and safe mandibular anesthesia is performed. For this purpose, the prepared device for mandibular anesthesia is placed in the patient's oral cavity within the maximum concavity of the front edge of the lower jaw branch until reaching the anvil. Device handle is located vestibularly from dental arches, moving angle of mouth and cheek outside to level of interocclusal height with open mouth, after which cartridge syringe with anesthetic and needle are installed parallel to device in area at premolars level from opposite side, and the needle is placed in the plane formed between the limiter plates, and brought to the furrow of the end cowl and advanced inside the tissues to achieve a needle insertion depth pre-set by the limiter, and the needle tip reaches the target point for administering an anesthetic, for this purpose, by moving the tissue to reach the bone, introducing anesthetic agent 0.3 ml in order to switch off the lingual nerve, then the syringe is moved to the frontal teeth and the needle is moved backwards, without losing contact with bone, until it stops in the pre-established limiter, in which the needle reaches the target point, followed by an aspiration test and creating an anesthetic depot. After the anesthesia is completed, the cartridge syringe is removed from the device, and then the mandibular anesthesia device itself. Anesthesia occurs within 5–10 minutes; the anesthesia zone corresponds to a standard technique. Use of the declared invention provides the conditions for increasing anesthetic effect and safety of anesthesia due to individual determination of distance between anterior edge of mandible branch and target point, by eliminating anatomy-topographical landmarks in maxillofacial area difficult-to-remember and complex for dentist.
EFFECT: similar approach creates preconditions of considerable minimization of injuries of mandibular neurovascular bundle, temporal and medial pterygoid muscles.
