FIELD: medicine.
SUBSTANCE: invention relates to medicine, specifically to ophthalmology. To form an ultrathin corneal graft for posterior layer-by-layer keratoplasty, transplant thickness is measured by optical coherence tomography (OCT), a section of the thickness of the donor cornea with microkeratome, repeated OCT pachymetry and exposure to an excimer laser. Cut of thickness of the donor cornea is made with microkeratome MoriaSLK2 or MoriaSLK3 with a turbine of longitudinal type head of caliber 450, 500 or 550 microns. It is followed by repeated OCT pachymetry to obtain a detailed map of thickness of excess corneal layers of the donor. Data are entered into the program of the excimer laser Microscan 500 or Microscan 1000 "Complex FTK" and act at wavelength 193 nm, pulse repetition frequency of 500 or 1,000 Hz, respectively, diameter of laser spot 0.9 mm, maximum diameter of ablation 10.9 mm. Transplant excess thickness is simultaneously evaporated in the central and peripheral regions so that the peripheral part evaporates in a greater volume than the central one, which results in a uniform transplant formation.
EFFECT: method enables preparing an ultrathin graft with no risk of rejection of the donor material with a planned thickness, wherein the adhesion of the donor material to the cornea of the recipient is improved and a hypermetropic shift is reduced, which increases visual acuity.
