FIELD: medicine; ophthalmology.
SUBSTANCE: invention can be used for preparing ultrathin grafts for posterior lamellar keratoplasty. To do this, the thickness of the graft is first measured using optical coherence tomography (OCT). The first cut of the donor cornea is made using a Moria SLK3 microkeratome with a longitudinal turbine with 450, 500 or 550 mcm calibre heads. Repeated OCT pachymetry is performed. The remaining corneal thickness is determined. Pachymetry data and residual thickness parameters are entered into the WaveLight EX500 excimer laser program. Further photoablation of the remaining donor tissue in the central and peripheral zones is performed using the photorefractive keratectomy (PRK) algorithm with the following laser parameters: wavelength 193 nm, pulse repetition rate 500 Hz, estimated time 19 s, optical zone 7.0 mm, ablation zone 7.6 mm, the depth of maximum ablation and the thickness of the residual stroma depend on the initial thickness and hydration of the graft.
EFFECT: invention makes it possible to prepare an ultrathin graft for PLK with minimal loss of endothelial cells, which has a positive effect on the course of the postoperative period, graft survival and the final visual acuity of the patient.
