SUBSTANCE: invention relates to a surgical instrument. Surgical instrument for treating a patient's tissue comprises a barrel assembly, an end effector and a staple cassette. End effector extends from the shaft assembly along the central line of the branch. End effector comprises a first branch and a second branch. First jaw has a stop configured to form staples pressed to the anvil. First and second jaws are configured to switch between open configuration and closed configuration. Staple cartridge is placed inside the second branch. Staple cartridge comprises a platform, a first end extractor assembly and a wedge-like slide. Platform faces the limit stop. Platform forms holes for staples. Brackets are located inside staple holes. First unit of the end extractor is located adjacent to the distal end of the staple cartridge. First unit of the end pusher is configured to receive the first end portion of the staples. First unit of the end extractor at least partially forms under itself space for storage inside the second branch. Wedge slides are made with possibility to slide near platform from proximal position of slides in distal position of slides and engagement of first unit of end extractor, and forced movement of end part of staples to support for formation in fabric. At least a greater part of the length of the wedge-like slides is configured to be placed in the storage space, without sliding in the distal direction beyond the first unit of the end extractor, when wedge-like slides are in distal position of slide. Wedge-like slides comprise the first cam inclined surface made with possibility of engagement with the first unit of end extractor. First cam inclined surface comprises front cam surface and intermediate cam surface. Front cam surface forms the first inclination angle between the lower surface and the front cam surface. Intermediate cam surface forms second inclination angle between lower surface and intermediate cam surface. First angle of inclination of the front cam surface is more, than the second angle of inclination of the intermediate cam surface. Extractor assembly is configured to engage with the front cam surface to lift the ejection assembly upward at a first upper distance to the first branch at a first distal distance. Ejector assembly is made with possibility of engagement by intermediate cam surface for additional lifting of ejector assembly upwards to second upper distance to first branch to a second distal distance to provide sufficient force for pressing end part of staples to a stop for use in tissue. First upper distance is greater than second upper distance.
EFFECT: using the invention enables tissue cross-linking and dissection in the patient's body.
