FIELD: medicine.

SUBSTANCE: invention refers to medicine, namely to traumatology and orthopedics, and can be used for restoration of acromioclavicular junction by re-surgical acromioclavicular ligament with application of biodegradable anchor fixator and hook-like plate. Prior to surgical treatment, X-ray imaging and ultrasonic diagnostics are used to determine spatial visualization of involved acromioclavicular joint structures. Surgical intervention in the patient's position is performed by a semi-incision of a longitudinal incision of 4-5 cm of the skin and subcutaneous fat in a projection of the acromioclavicular joint. Acromioclavicular joint is acutely and blunt accessed, the hematoma is evacuated if present, the acromion process of the blade, the acromial end of the clavicle and the injured acromioclavicular ligament are visualized. Acromioclavicular ligament of the scar-modified ligament is excised. Then, an anchor fixture equipped with biodegradable synthetic material is fitted on the external surface of the acromial end of the clavicle. Acromioclavicular joint is dislocated and the acromioclavicular ligament is reinserted by suturing with U-sutures using an anchor fixation filaments. Hook of the hook-like plate is placed under the acromial process of the blade and a hook-shaped plate is fixed under the electron-optical control on the external surface of the acromial end of the clavicle using three fixation screws made of titanium alloy BT6. Wound is layer-by-layer wound closure. Hook-shaped plate is removed 4-6 months after the surgical intervention. Anchor fixator made of a biodegradable synthetic material based on a lactic and glycolic acid copolymer (PLGA) is placed on the external surface of the acromial end of the clavicle.

EFFECT: method provides stiffness of fixation of acromioclavicular joint, prevention of postoperative pyoinflammatory complications, anatomical restoration of the shape and function of the structures of the acromioclavicular joint, prevention of posttraumatic arthrosis by using a biodegradable anchor fixator and a hook-like plate.
