SUBSTANCE: invention refers to medicine, particularly to oncology. Disclosed is a method for selecting a therapeutic approach to patients with regional lymphadenopathy of unclear genesis in gastric adenocarcinoma. A patient with histologically verified gastric adenocarcinoma is examined by immunohistochemical examination of the biopsy material to determine the level of the proliferative index Ki-67. When Ki-67≥45%, regardless of the degree of quantitative and qualitative lymphadenopathy, the patient is referred to a high risk of metastasis, and neoadjuvant polychemotherapy is performed at a preoperative stage. If Ki-67<45% surgical intervention is performed without preliminary neoadjuvant polychemotherapy provided no other indications for polychemotherapy.

EFFECT: invention provides determining the patient's management approach to prescribing the neoadjuvant polychemotherapy at the preoperative stage in the patients diagnosed with gastric adenocarcinoma and regional / perigastric lymphadenopathy of unknown origin.
