FIELD: medicine.

SUBSTANCE: invention relates to medicine, namely, to oncology, and pertains to methods for combined treatment of locally advanced cardioesophageal cancer. Chemotherapy courses are conducted according to the regimen including the introduction of oxaliplatin 100 mg/m2, leucovorin at a dose of 400 mg/m2 by intravenous drop infusion on 1 day, the duration of infusion 2 hours, 5-fluorouracil at a dose of 400 mg/m2 by intravenous jet infusion on 1 day. Then at a dose of 2,400 mg/m2 intravenously, the duration of infusion 46 hours. The break between the chemotherapy courses is 2 weeks. In the amount of 6 courses and simultaneously with chemotherapy, radiation therapy is conducted in the mode of standard fractionation of a single boost dose equal to 2 Gy, a total boost dose equal to 46 Gy, 5 times a week. Irradiation is conducted with high-energy photons on a linear electron accelerator, conformal irradiation is applied, the duration of the irradiation course, accounting for weekends, is 31 days. Chemotherapy is also conducted in a neoadjuvant mode, wherein oxaliplatin is introduced at a dose of 100 mg/m2. The irradiation zone includes the abdominal segment of the esophagus, the cardiac section of the stomach, lower mediastinal, paraesophageal, perigastric lymph nodes, lymph collectors in the area of the celiac trunk and branches thereof. The duration of the irradiation course, accounting for weekends, is 31 days. 6 weeks after the completion of preoperative radiation therapy, the tumour response is assessed and surgical intervention is performed in the volume of gastrectomy.

EFFECT: method improves the results of combined treatment of locally advanced cardioesophageal cancer by means of combined application of radiation and chemotherapy.
