FIELD: medicine.
SUBSTANCE: invention relates to medicine, namely to traumatology, and can be used to replace extensive diaphyseal defects of long bones. The method includes preoperative planning, namely the design of an implant model based on CT examination of the bones of paired limbs. On the basis of the data obtained, a monolithic implant is formed from a titanium alloy with a proximal part represented by a cannulated round pin with a channel, the ratio of the channel diameter to the pin diameter is 0.4-0.45, with holes for fixing elements, a diaphyseal anatomically curved part replacing the bone a defect, in the form of a sleeve, and a distal part made in the form of a lanceolate plate with an anatomically curved distal expanding part with holes for fixing elements, having a porous trabecular structure on the surface. The ratio of the lengths of the proximal part of the implant to the distal part of the implant and to the diaphyseal part is 1 : 1 : 1.3. During the integration of the implant, the proximal part of the long tubular bone is drilled out with the installation of the cannulated implant post. The pin is blocked through the holes with fixing elements. The implant plate is fixed in the distal part of the long tubular bone using bone grafting in the places where the porous trabecular structure of the implant adjoins the bone.
EFFECT: method allows to increase the efficiency of treatment of extensive diaphyseal defects of long tubular bones of various etiology due to preoperative planning and formation of an individual implant.
