FIELD: medicine; traumatology and orthopaedics.
SUBSTANCE: invention can be used when performing revision hip arthroplasty in case of Paproksy IV type of femoral bone defect. Preoperative planning, computed tomography of the affected and contralateral limb segments, modelling and printing of the implant on a 3D printer and installation of the implant on cement are carried out. The implant contains a proximal module and a stem. The implant is modelled according to the parameters of the contralateral segment so that the height of the proximal cylindrical module with the neck corresponds to the distance from the lesser to the greater trochanter, its diameter corresponds to the healthy bone diameter at the level of the lesser trochanter, the position of the neck corresponds to the offset and cervical-diaphyseal angle of the contralateral limb. The implant stem is modelled in such a way that its distal end is at least 1 mm higher than the Blumensaat line. The proximal end of the implant stem, when immersed in the bone, provided the formation of a cement mantle of at least 2 mm. A mesh is modelled over the entire surface of the implant stem, and Hirth's joint is modelled at the point of contact between the ends of the proximal module and the stem. After the implant is installed on the bone cement, a cortical bone allograft of the tibia is placed along the outer surface with an approach to the distal part of the femur and fixed with cerclage sutures.
EFFECT: method provides reduction of trauma risk and restoration of joint geometry and total limb length with reliable fixation of the operation due to removal of only damaged bone structures.
