FIELD: medicine.
SUBSTANCE: invention relates to medicine, namely to phthisiology. In patients with a diagnosed tuberculous lesion of the pleura, the data of radiation research methods, including computed tomography of the chest organs, as well as ultrasound data of the pleural cavity and clinical and laboratory symptoms, are evaluated. At the same time, in the presence of single or multiple pleural loculations, the absence of free fluid in the pleural cavity, the absence of dominant tuberculosis processes of other localization and the absence of pronounced clinical and laboratory symptoms of tuberculosis inflammation, surgical treatment is carried out before the start of anti-tuberculosis therapy. In the absence of positive X-ray dynamics from the antitubercular therapy with the formation of pleural loculations or armored thickening of the pleura, in the absence of free fluid in the pleural cavity, no tendency to its repeated accumulation, in the absence of dominant tuberculosis processes of other localization, with a decrease in the severity of clinical and laboratory symptoms, surgical treatment is carried out 2-3 months after the start of antitubercular therapy. And in the presence of pleural loculations or armored thickening of the pleura in combination with the dominant tuberculous process of another localization in the absence of free fluid in the pleural cavity, surgical intervention is carried out after 6 or more months, determining the period individually when the dominant tuberculous process is stabilized according to clinical and laboratory symptoms and radiation studies.
EFFECT: method makes it possible to increase the accuracy of determining the timing of surgical treatment for tuberculous pleural empyema in children and to expand the arsenal of technical means that can be used for these purposes.
