SUBSTANCE: invention relates to medicine, namely to maxillofacial surgery. A bony window of an ovoid shape is formed in the anterior wall of the upper jaw on the side of the formation of the future flap towards the defect. The recipient bed is prepared by mobilizing the oral mucosa adjacent only to the distal part of the defect. The free edges of the mucous membrane are sutured together with interrupted and U-shaped sutures along the median line of the defect. In this case, the proximal part of the defect remains uncovered for the passage of the vascular pedicle. A mucous-muscular flap on the upper base of the facial artery with a reverse blood flow is used as a soft tissue flap to eliminate a defect in the anterior part of the hard palate. The flap is rotated upward and passed through the formed window of the maxillary sinus to the defect area in the anterior hard palate. The flap is fixed to the edges of the recipient bed.

EFFECT: method allows increasing the functional and aesthetic effect of surgical treatment, to use this modification of the mucous-muscular flap on the facial artery for the reconstruction of penetrating defects in the anterior hard palate in young patients without dentition defects, to reduce the risk of trauma.
