SUBSTANCE: invention relates to medicine, namely to dentistry, and can be used for gum transplantation for functional and aesthetic indications. The guide template for soft tissue transplantation is a monolithic aligner made by computer modeling and additive manufacturing from a bioinert medical polymer based on a volumetric model of the patient's upper jaw obtained by combining computed tomography data and optical scanning of the jaw. In this case, the template overlaps the teeth from the vestibular side at the level of the clinical equators and completely overlaps the palate. The template has an opening of the shape determined at the stage of clinical planning of the operation corresponding to the geometry of the operating field. The thickness of the walls of the guide template along the contour of the hole corresponds to the thickness of the patient's mucosa, determined from the data of computed tomography and scanning of teeth, and is made with the possibility of adjusting the depth of immersion of the scalpel blade in the donor zone.

EFFECT: template allows providing a perpendicular direction of the scalpel during the extraction of the gingival graft at a given depth with a certain shape, to reduce trauma, and also to shorten the time of surgical intervention.
