FIELD: medicine.
SUBSTANCE: invention relates to medicine, namely to surgical dentistry, and can be used during gum transplantation. To do this, the patient undergoes a computed tomography of the jaws and scanning of the dentition at the planning stage of dental treatment. The obtained images are combined in the module of a computer program for modeling dentures and navigation patterns-mouthguards are simulated that overlap the teeth and reach the transitional fold of the alveolar bone mucosa, determining the route of mouthguard insertion, isolating undercuts on three-dimensional models of the tooth ranges. In the area of the proposed operation, the semilunar shape of the future incision is contoured for subsequent gum transplantation and a hole in the mouthguard is cut out along this contour, a hole similar in shape and size is cut out on the mouthguard in the area of the donor site on the palatine surface of the upper jaw in the position of premolars and molars. After making sure on a CT scan that the specified hole does not intersect with the large palatine opening of the upper jaw, through which nerves and vessels exit into the submucosal layer, virtual models of mouthguards are translated into physical ones by means of an additive computer method for producing 3D printing from a dental polymer. The mouthguards are subjected to post-printing processing and sterilization, anesthesia is performed in the operation area and the donor gum sampling area. A mouthguard is fixed on the dentition and a gum incision is made to the periosteum along the contour of the previously formed mouth guard opening facing the necks of the teeth. Then, the muscles are moved apically to the lower edge of the contour of the hole in the mouthguard with a raspatory. A mouthguard is applied to take the donor gum and an incision is made along the contour of the previously formed hole in it, the mouthguard is removed, the gum is split and the epithelized free flap is taken away. The resulting flap is placed on the surgical wound and sewn with nodular sutures to the edges of the gum, the donor gum is sewn to the periosteum with mattress knots.
EFFECT: method provides high-precision atraumatic sampling of donor tissue for gum transplantation in the shape and size corresponding to the operating field.
