FIELD: medicine.
SUBSTANCE: invention relates to medicine, namely to traumatology and orthopedics, and can be used to treat patients with posterolateral rotational instability of the knee joint with combined damage to the posterior cruciate ligament and hamstring tendon. A tendon allograft is performed to replace the posterior cruciate ligament through bone tunnels in the tibia and femur and it is fixed on the femur with a bone plate. Then a through tibial bone tunnel and a blind femoral bone tunnel are formed. The graft from the long fibular muscle is inserted into the formed blind femoral bone tunnel, where it is fixed with an interference screw, then the graft is passed in the joint behind the posterior horn of the lateral meniscus through the formed tibial bone tunnel parallel to the usual location of the hamstring tendon. Then, with an interference screw, a previously installed tendon allograft is fixed to replace the posterior cruciate ligament in the tibial tunnel at an angle of flexion in the knee joint 90° with simultaneous elimination of the posterior subluxation of the lower leg, and an allograft to replace the hamstring tendon is fixed in the tibial bone tunnel with an interference screw when bending the knee joint at an angle of 90° and maximum internal rotation of the lower leg.
EFFECT: method makes it possible to improve the outcome of surgical treatment, reduce the risk of intraoperative neurological complications, expand the possibilities of arthroscopic visual control of all stages of the operation without the formation of additional arthroscopic ports and improve the functional result due to simultaneous reconstruction of the posterior cruciate ligament and popliteal muscle tendon using allotendon material using arthroscopic technique and periosteal femoral graft fixation for the posterior cruciate ligament.
