FIELD: medicine.
SUBSTANCE: invention refers to medicine, namely to traumatology and orthopaedics, and can be used for surgical treatment of impression deformation and fracture of posterior lateral condyle of tibia. Fibular bone is mobilized by means of a tibial bone osteotomy with an osteotomy plane parallel to the articular surfaces of the tibiofibular articulation, cutting it off with an osteochondral base so that the entire tibiofibular articulation with a ligamentous apparatus is separated from the tibia. Tibiofibular articulation with the ligamentous apparatus together with the fibular bone is retracted posteriorly, and the lateral condyle of the femur is retracted upward by a retractor. Subchondral osteotomy of the tibia is performed through the formed gap. Osteochondral fragment including the whole impression area is dissected away. It is lifted to restore the level of the articular surface; formed triangular-trapezoidal bone defect is measured. It is filled with a cortical spongy autograft equal to it in shape and size from an iliac wing. Fibular bone with the anterior wall of the base of the tibiofibular articulation is repaired and fixed with screws with compression to the tibia through the tibiofibular articulation parallel to the level of deformation elimination.
EFFECT: method provides restoration of the articular surface of the posterior lateral condyle of the tibia and adequate replacement of the bone defect, as well as the stability of the fragments fixation without the use of the bone plate due to the fact that the repositioned impression fragment is retained by the graft, which lies on the spongy bone of the tibial epiphysis and rests with its posterior edge on the articular surface of the fibular bone, in which, when the screws are tightened, a component force directed upwards occurs, which enhances resistance to secondary impression by occurrence of two supporting effects from graft and head of fibular bone.
