METHOD FOR TREATMENT OF PATIENTS WITH MUSCULOSKELETAL DYSFUNCTION OF THE TEMPOROMANDIBULAR JOINT Russian patent published in 2021 - IPC A61C7/00 A61C7/36

Abstract RU 2762332 C1

FIELD: medicine.

SUBSTANCE: invention relates to the field of medicine, namely orthopedic dentistry, and can be used at the initial therapeutic and diagnostic stage to achieve optimal balanced movement of the lower jaw, to prevent bite displacement in the course of complex treatment of patients with temporomandibular joint (TMJ) dysfunction. To do this, the optimal height of the lower part of the face and the therapeutic position of the lower jaw are determined by separating the dentition and kinesiological actions. Next, a kinesiography examination is performed. Then, by means of heated wax rollers, the optimal height of the bite position is determined and the desired position of the lower jaw is obtained. Then kinesiological tests start to assess the mobility of the cervical spine. The next step is to remove imprints from the first and second rollers for the manufacture of working models from a hardness class IV die stone, on which the landmarks of the labial bridles in the form of lines are applied with a marker and installed in the articulator space using a facial arc and wax rollers fixing the therapeutic position of the jaws in the articulator space. After that, plaster models of the jaws are used to make an intraoral occlusal apparatus. The resulting intraoral occlusal apparatus is stored on the dentition of the lower jaw, using articulation paper to obtain prints of occlusal contacts on the occlusal surface of the intraoral apparatus. Longitudinal front and rear occlusal tracks are created on the chewing surface of the device to obtain a front guide and a rear balancing support. The anterior and lateral parts of the surface of the intraoral occlusal apparatus are sanded. When the stabilization of occlusion and centering of the jaws on the intraoral occlusal apparatus is achieved, the blocking of movements on the side of condyle compression is eliminated, working side movements are restored, the patient adapts to the optimal height of the lower face and when restoring the function of the dental system, orthodontic or orthopedic treatment is carried out, continuing the use of the intraoral occlusal apparatus.

EFFECT: method provides effective treatment of musculoskeletal dysfunction of the temporomandibular joint by stabilizing the optimal position of the jaws due to the creation of a canine guide in harmonious interaction with the distal balancing on the intraoral occlusal apparatus, obtaining an occlusal diagonal with simultaneous occlusal functioning, canine sliding on the left and balancing contact on the distal buccal cusp of the second molar on the right and posterior balancing support.
