FIELD: medicine.
SUBSTANCE: invention relates to medicine, namely thoracic surgery, and can be used to treat patients with perforations of the thoracic esophagus. After the diagnosis of esophageal perforation, surgical access is performed with the preservation of the anterior dentate and broadest muscles. Myoplasty of the esophageal wall defect is performed with a vascularized muscle flap, fixing it with separate nodular sutures with the installation of drains from the pleural cavity for passive outflow. After that, an intraluminal vacuum aspiration system (VAC system) is installed. Next, the vacuum aspiration system is changed until the defect is eliminated and there is no leakage of contrast beyond the esophagus according to X-ray.
EFFECT: method makes it possible to increase the effectiveness of treatment of esophageal perforations and ensures the elimination of contact of the VAC system with elements of the mediastinum and pleural cavity due to a combination of myoplasty of the esophageal defect and intraluminal VAC therapy.
