SUBSTANCE: invention relates to medicine, namely to abdominal surgery and can be used to treat perforations of the esophagus. An endoscope with a distal nozzle is inserted into the lumen of the esophagus. Esophagoscopy is performed. A defect in the wall of the esophagus is revealed. An electrosurgical knife is passed through the endoscope channel. An endoscope with a distal nozzle is passed to the distal edge of the defect. Then, using an electrosurgical knife, the esophageal wall is dissected in the distal longitudinal direction, lengthening the defect to 1.5-3 cm, in a mixed mode of cutting and coagulation. Hemostasis of the dissected edges of the wall is carried out. The endoscope is passed through a dissected defect in the wall of the esophagus into the pathological cavity of the mediastinum. Installation of a system for endoscopic vacuum therapy in the pathological cavity of the mediastinum is performed.

EFFECT: method makes it possible to form a wide intraluminal access to the pathological mediastinal cavity for endoscopic vacuum therapy with defects of the esophagus less than 1 cm with minimal impact on the surrounding structures, to simplify the placement of a polyurethane sponge for endoscopic vacuum therapy, to ensure adequate drainage and sanitation of the pathological cavity, to accelerate the process of regeneration and reduction cavity, isolate the mediastinal cavity from the contents of the esophagus, shorten the postoperative period.
