METHOD FOR PREDICTING RECURRENCE OF PULMONARY TUBERCULOSIS Russian patent published in 2021 - IPC G01N33/68 G01N33/564 G01N33/573

Abstract RU 2762671 C1

FIELD: medicine.

SUBSTANCE: invention relates to medicine, namely to laboratory studies in phthisiology; it can be used to predict the recurrence of pulmonary tuberculosis by examining blood serum. In patients with pulmonary tuberculosis caused by drug-resistant strains of M.tuberculosis, concentrations of interferon-γ, interleukin-10, autoantibodies to type I collagen and autoantibodies to type III collagen are determined in the midst of the disease by the ELISA method, and the high, medium, low risk of the recurrence or the complete absence of this risk is determined, according to the proposed algorithm. Step A: the concentration of interferon-γ is determined: at its value of 5.59 pg/ml and above, 0 points are assigned; at its value of 4.58-5.58 pg/ml, 1 point is assigned; at its value of 3.58-4.57 pg/ml and above, 2 points are assigned; at its value of 3.57 pg/ml and below, 3 points are assigned. Step B: a value is added to the obtained point value depending on a certain concentration of interleukin-10: at its value of 5.0 pg/ml and above, 0 points are added; at its value of 3.26-4.9 pg/ml, 1 point is added; at its value of 2.25 pg/ml and below, 2 points are added; at its value of 2.25-3.25 pg/ml, 3 points are added. Step C: a value is added to the obtained point value depending on a certain concentration of autoantibodies to collagen I: at its value of 7.1 mcg/ml and below, 0 points are added; at its value of 7.2-12.41 mcg/ml, 1 point is added; at its value of 13.43 mcg/ml and above, 2 points are added; at its value of 12.42-13.42 mcg/ml, 3 points are added. Step D: a value is added to the obtained point value depending on a certain concentration of autoantibodies to collagen III: at its value of 20.01 mcg/ml and above, 0 points are added; at its value of 12.18-20.0 mcg/ml, 1 point is added; at its value of 11.16 mcg/ml and below, 2 points are added; at its value of 11.17-12.17 mcg/ml, 3 points are added. Step E: Prediction of relapse risk: with a total score of 10-12, a high risk of recurrence is predicted; with a score of 7-9, the average risk of recurrence is predicted; with a total score of 4-6, a low risk of recurrence is predicted; with a score of 0-3, no risk of recurrence is predicted.

EFFECT: method makes it possible to predict the recurrence of pulmonary tuberculosis caused by drug-resistant strains of M.tuberculosis by taking into account peculiarities of collagen metabolism disorders in these patients, which makes it possible to timely correct pathogenetic therapy aimed at normalizing collagen metabolism, reducing the massiveness and prevalence of residual changes in lung tissue, thereby increasing the efficiency of treatment of tuberculosis patients.
