SUBSTANCE: invention relates to medicine, namely to surgery. Access to the antero-superior mediastinum is performed by cervicotomy according to Kocher. Allocate bluntly on both sides and mobilize thyrothymic ligaments. Billroth clamps are applied to the distal parts of the thyrothymic ligaments and these parts are separated from the lower poles of the thyroid gland. For the imposed Billroth clamps, one-step bilateral upward traction of the thyrothymic ligaments is performed with simultaneous mobilization and removal from the upper mediastinum to the neck of the upper thymus horns with ectopic lower parathyroid glands. The isolated lower parathyroid glands are grasped with fenestrated Luer clamps and removed.

EFFECT: method allows expanding the possibilities of surgical intervention in hyperparathyroidism with an ectopic location of the lower parathyroid glands in the anterior-superior mediastinum, increasing the efficiency of surgical treatment, reducing the invasiveness of access, shortening the time of postoperative rehabilitation, eliminating the need for repeated surgical intervention.
