Abstract RU 2773275 C1

FIELD: medicine.

SUBSTANCE: invention relates to medicine, namely to medical microbiology, dentistry and periodontology, and can be used to predict the severity of periodontitis by the composition of conditionally periodontopathogenic species of the microbiome of the root of the tongue. Identification of conditionally pathogenic microorganisms of the human body is carried out by conducting a microbiological study. For this purpose, biomaterial is taken from the surface of the tongue, making a wide zigzag swab from the surface of the mucous membrane along the width and length of the middle third, the back, and the back third, the root of the tongue with a sterile viscose tampon on a plastic probe. Semi-quantitative sowing is done on 5% blood agar, and with the same tampon, primary sector sowing is done, with strokes within the boundaries of the sector, on lamellar chromogenic nutrient media in Petri dishes with a diameter of 90 mm: UriSelect 4, mannitol-salt agar, enterococcus-agar, Endo and Saburo agar. It is cultivated in a thermostat at a temperature of 35°C. Up to 20 strokes are performed with sector sowing, for 1/2 cup with a nutrient medium. In case of detection of gram-positive cocci in the biotope of the root and back of the tongue, oral greening streptococci of the ecological group S. viridans, in the titer lgKOE≤5, which includes strains of S. mitis, S. salivarius, S. sanquis; non-pathogenic neisseria and corynebacteria without titer normalization, single colonies of 0-6 staphylococci and/or enterococci in the lgKOE≤3 titer, lgKOE≤4 leptotrichia, the result is regarded as a normocenosis of the microecological niche of the oral cavity with a prognosis of no risk of aggravation of local periodontitis and/or exacerbation of the chronic process in periodontal tissues. In case of detection of gram-positive cocci in the biotope of the root and back of the tongue, oral greening streptococci of the ecological group S. viridans in the titer lgKOE≥5, non-pathogenic neisseria, corynebacteria without titer normalization, Enterococcus spp. in the title lgKOE≤5, Staphylococcus spp. in the titer lgKOE≤4, leptotrichia Leptotrichia spp. lgKOE≤5, the result is regarded as an initial dysbiosis of the microecological niche of the oral cavity with a prognosis during periodontitis of mild or moderate severity without aggravation and/or exacerbation of the chronic infectious process. In case of detection of the root and back of the tongue from the biomaterial, along with oral greening streptococci of the S. viridans group in any lgKOE≥5 titer, non-pathogenic neisseria, corynebacteria without titer normalization, Enterococcus spp. in the title lgKOE≥5, Staphylococcus spp. in the titer lgKOE≥5, leptotrichia Leptotrichia spp. lgKOE≥6, the result is regarded as a dysbiosis of the microecological niche of the oral cavity of moderate severity with a prognosis of the risk of aggravation of the infectious process during periodontitis of mild severity to moderate severity, moderate severity to severe and/or the risk of exacerbation of the chronic process with a recurrence rate of 1-4 times a year. In case of identification in the biomaterial from the root and back of the tongue along with oral greening streptococci of group S. viridans in any lgKOE≥5 titer, non-pathogenic neisseria, corynebacteria without titer normalization, Enterococcus spp. in the titer lgKOE≥5, leptotrichia Leptotrichia spp. lgKOE≥5, Candida albicans in the titer lgKOE≥5, the result is regarded as dysbiosis of the microecological niche of the oral cavity of moderate severity with a prognosis of the risk of aggravation of the infectious process during candida-associated periodontitis of mild severity to moderate severity, moderate severity to severe and/or the risk of exacerbation of the chronic process with a recurrence rate of 1-4 times a year. In the case of detection from the biomaterial of the root and back of the tongue, along with oral greening streptococci of the S. viridans group in any lgKOE≥5 titer, non-pathogenic neisseria, corynebacteria without titer normalization, Staphylococcus aureus in the lgKOE≥5 titer, Leptotrichia Leptotrichia spp. lgKOE≥6, Streptococcus pyogenes in the lgKOE≥6 titer, the result is regarded as a dysbiosis of the microecological niche of the oral cavity of moderate severity with a prognosis of the risk of aggravation of the infectious process during periodontitis of moderate severity to severe and/ or the risk of exacerbation of the chronic process with a recurrence rate of 2-4 times a year. In case of identification from the biomaterial of the root and back of the tongue along with oral greening streptococci of group S. viridans in any lgKOE≥5 titer, non-pathogenic neisseria, corynebacteria without titer normalization, Enterococcus faecalis in lgKOE≥6 titer, Leptotrichia spp. in any titer, the association of enterococci with one of the bacterial species of the Enterobacterales family, such as Klebsiella pneumoniae in the lgKOE≤5 titer; or Escherichia coli in the lgKOE≥4 titer; or Enterobacter cloacae lgKOE≥4 the result is regarded as severe oral dysbiosis with a prognosis of the risk of aggravation of the infectious process during periodontitis of moderate severity to severe and/or the risk of exacerbation of the chronic process with a recurrence rate of 2-4 times a year. In the case of detection from the biomaterial of the root and back of the tongue, along with oral greening streptococci of the S. viridans group in any lgKOE≥5 titer, non-pathogenic neisseria, corynebacteria without titer normalization, Enterococcus faecalis in the lgKOE≥6 titer, or Enterococcus faecium in the lgKOE≥5 titer; Leptotrichia spp. in any titer, the association of enterococci with one of the types of non-fermenting gram-negative bacteria, such as Acinetobacter baumannii in the lgKOE≥4 titer or Pseudomonas aeruginosa in the lgKOE≥4 titer, the result is regarded as severe oral dysbiosis with a prognosis of the risk of aggravation of the infectious process during periodontitis of mild severity to moderate-severe, moderate-severe to severe and/or risk of exacerbation of the chronic process with a recurrence rate of 1-4 times a year.

EFFECT: method provides the possibility of indirect prediction of the severity and exacerbation of nonspecific periodontitis due to the isolation of indigenous conditionally pathogenic oral and nonoral microorganisms from the dorsal surface of the mucous membrane of the tongue by standard microbiological methods, with the interpretation of the composition and titer indicators of conditionally periodontopathogenic types of the microbiome of the tongue.
