SUBSTANCE: invention relates to medicine, namely surgery and transplantation. The graft is removed from the transport container. The artery is cannulated, through which oxygenated cold perfusion is carried out using a perfusion pump connected to an oxygenator. The perfusion system is filled with a solution of Custodiol cooled up to 4°C. The graft after cannulation of the artery is placed in a a solution of Custodiol cooled to 4°C located in a metal container. The perfusion pump pumps perfusate through the artery of the graft, which is poured through a vein into a metal container, from where it is taken into a closed perfusion system. Every 30 minutes, the perfusate pouring out of the vein is taken for analysis to determine the acid-base balance. Pre-transplantation preparation of the graft begins in parallel with surgical access to reduce the risk of heat damage. After the formation of vascular anastomoses and the start of blood flow, a biopsy is also taken.

EFFECT: method makes it possible to improve the immediate and long-term results of liver and kidney transplantation, reducing the risk of graft dysfunction by using cold oxygenated perfusion as a method for organ preservation.
