FIELD: medicine.
SUBSTANCE: invention refers to medicine, namely to transplantology, and can be used to determine early dysfunction of the reticuloendothelial system (RES) of the liver in patients following orthotopic transplantation with no signs of graft dysfunction and manifested cytolysis by biochemical data. In patients with LDH, AST, ALT, bilirubin exceeding the upper limit of normal values by no more than 15–20 %, tacrolimus concentration in peripheral blood is determined. Whole body and liver are scanned in anterior and posterior projections 20–30 min after single administration of 99mTc-Technephyt. Total pulse count of the whole body and a mean hepatic count are determined. Obtained parameters are used to determine specific capture of 99mTc-Technephyt by liver from whole body pulse counting taking into account tacrolimus concentration in a specific patient by formula: SCHWBTc = (H1/B1 + H2/B2) × 5000 + K × Tc, where H1 is the average count of liver pulses on the scintigram in the anterior projection, B1 - total count of whole body pulses on a scintigram in a front projection, H2 - average count of liver pulses on a scintigram in a posterior projection, B2 is the total count of whole body pulses on the scintigram in the posterior projection, Tc is tacrolimus concentration in the peripheral blood, ng/ml, K = 0.09 is the experimentally calculated coefficient taking into account 99mTc-Technephyt capture by the transplanted liver with no immunosuppressive therapy. If observing values 1.63 ≤ SCHWBTc ≤ 1.83, the conclusion is made on the absence of RES transplant dysfunction. When obtaining the SCHWB values of beyond the specified confidence interval, a conclusion is made on early dysfunction of the RES transplant.
EFFECT: method provides early diagnosis of transplant dysfunction by evaluating the accumulation of a radiopharmaceutical in the reticuloendothelial cells of the liver taking into account the concentration of tacrolimus or its analogues in the patient's peripheral blood.
