SUBSTANCE: invention relates to medicine, namely to traumatology and orthopedics, and can be used to perform an angle-opening high tibial osteotomy in the treatment of medial gonarthrosis. Before performing osteotomy, 2 Kirschner wires with a diameter of 2 mm are performed. In AP view, the pins extend 2.5 mm below the tibial plateau from the medial cortical and terminate at the apex of the fibula on the lateral cortical. In this case, the spokes are carried out parallel to the proposed osteotomy zone and parallel to each other. Below the spokes by 0.5 cm and parallel to them, an osteotomy of the tibia is performed to the center of rotation of the corrective osteotomy. The needles are left in the bone. After osteotomy with implantation of a tricalcium phosphate block, the pins are removed, and the osteotomy area is fixed with a plate with screws.
EFFECT: method provides optimization of treatment of varus gonarthrosis by the method for high corrective osteotomy and prevention of intraoperative unstable fracture of the external tibial cortex, as well as reducing the risk of postoperative complications and reducing the time of surgical intervention by creating a mechanical obstacle to the line of intra-articular fracture of the osteotomy zone and selecting the osteotomy center of rotation.
