FIELD: medicine.

SUBSTANCE: invention refers to medicine, namely to traumatology and orthopaedics, and can be used for distalisation of tibial tuberosity in treating chronic lateral patellar instability. Before performing the operation, the volume of the proposed distalisation of the tibial tuberosity is determined based on the MRI and RG parameters, and the Caton-Deschamps index is measured. Medial and lateral zones of the proposed osteotomy are marked with an electrocoagulator; an osteotomy of the tibial tuberosity is performed at depth of 1–1.5 cm. Distalisation is performed on the basis of preoperative planning using measurement of Caton-Deschamps index, till anatomical position of patella is achieved. Anatomical position is confirmed by postoperative measurement of the Caton-Deschamps index, which normally should exceed 1. A tibial tuberosity is temporarily fixed with the help of two parallel K-wires. Range of motion in the knee joint, the position and stability of the patella are checked, and its tracking is assessed. Fixation is completed with 3.5 mm cortical screws.

EFFECT: method provides restoring the anatomical relationship in the patellofemoral joint, restoring the height of the patella according to the anatomical norm, allows to optimize patellar tracking in distalisation of tibial tuberosity due to the fact that osteotomy of tibial tuberosity is planned and its distalisation is performed by calculating the height of patella.
