SUBSTANCE: invention relates to medicine, namely to surgery and orthopedics, and can be used to replace total and subtotal foot defects in Charcot arthropathy. At the first stage, resection of deformed and purulent bones of the middle part of the foot and ankle joint is performed, the diastasis zone is sanitized with the removal of scar tissue, pathological granulation tissue. The foot is fixed with the preservation of the diastasis zone and the anatomically correct ratio of the length of its segments with an external fixation device. A spacer with an antibiotic is integrated into the diastasis. At the second stage, the spacer is removed and the resulting defect is replaced with an autograft taken from the femoral canal with the Rimmer Irrigator Aspirator Synthes system and mixed with the allograft in a ratio of 1:1. Fixation of the foot with an external fixation apparatus is continued until signs of consolidation of the bone defect of the foot are detected on radiographs.

EFFECT: method ensures the elimination of a purulent-destructive focus and the defect-diastasis of the foot sections that has arisen against this background, the restoration of the support ability of the foot, its length, as well as the preservation of its functionality due to the creation of bone ankylosis due to the phased implementation of arthrodesis.
