SUBSTANCE: invention relates to medicine, namely to traumatology and orthopedics, and can be used to replace total and subtotal foot defects in Charcot's neuroosteoarthropathy. At the first stage, resection of the deformed, destructively altered and purulently affected bones of the middle part of the foot and ankle joint is performed, the diastasis zone is sanitized with the removal of scar tissue, pathological granulation tissue and the foot is fixed in a functionally correct position with an external fixation device, the spacer is integrated with an antibiotic. At the same time, in the postoperative period, computed tomography of the operated segment is performed with the creation of a 3D model of the implantable spacer, followed by milling from the prepared bone block of the allobone of the workpiece according to the previously created 3D model. At the second stage, the spacer is removed with replacement of the defect with a prepared allograft, with continued fixation of the foot with an external fixation device until the X-ray picture of bone graft consolidation is performed.

EFFECT: method provides reduction of postoperative healing time, early restoration of the function of the proximal interphalangeal joint and reduction of the risk of deformity recurrence due to fixation techniques and use of a spacer.
