FIELD: medicine.
SUBSTANCE: invention refers to medicine, namely to traumatology and orthopaedics, and can be used for surgical treatment of heel bone fractures consolidated in the position of displacement with posttraumatic deformations of hindfoot. At the stage of preoperative planning, X-ray imaging, MSCT examination of the foot and ankle joint of a healthy and injured extremity are performed. Shape, size, volume of the calcaneal bone, anatomical reference angles of the hindfoot on a healthy extremity are assessed. Measured values are used to plan the extent of the reconstructive intervention on the injured contralateral extremity. Individual resection template is created, according to which osteotomies are performed. On the virtual preoperative planning, the results of the obtained parameters, relationships and spatial orientation of the calcaneal bone in the subtalar and calcaneocuboid joints are evaluated with the assessment of such parameters as the Meary angle, the angle of deflection of the talus, calcaneal inclination angle, Bohler angle, talocalcaneal height, calcaneal bone width in comparison with the counter-lateral side. At the stage of surgical intervention, an individual resection template is installed, according to which osteotomies are performed. Template is removed. Obtained calcaneal bone fragments are segmented, open reduction, subtalar arthrodesis with the use of immersed metal structures are performed.
EFFECT: method provides restoration of the structure of the injured calcaneal bone and hindfoot, close to the anatomically correct, preventing further progression of deformation, reduced pain syndrome, increased functional activity of the patient, improved cosmetic appearance of foot due to corrective osteotomy with the help of individual resection template of calcaneal bone and subtalar arthrodesis.
