METHOD FOR PREOPERATIVE PREDICTION OF THE RESULTS OF SELECTIVE NEUROTOMY OF THE MOTOR BRANCHES OF THE MUSCULOCUTANEOUS NERVE IN CHILDREN WITH CEREBRAL PALSY Russian patent published in 2022 - IPC A61B8/08 A61B5/00

Abstract RU 2779222 C1

FIELD: medicine.

SUBSTANCE: invention relates to medicine, namely, to physiology and orthopaedics, and can be used in preoperative prediction of the results of selective neurotomy of the motor branches of the musculocutaneous nerve in children with cerebral palsy. For this purpose, functional tests assessing the active amplitude of movements in the joints, the degree of arbitrary limb control, and bimanual skills are performed twice, before and after the diagnostic block of the musculocutaneous nerve with a prolonged-action anaesthetic agent. An electrophysiological examination of the condition of the upper limb of the patient is also performed. Said examination is performed with mandatory double verification of the nerve trunk under electrostimulation and ultrasound control. The obtained data are compared. If the amplitude of movements in the elbow, radiocarpal, and finger joints is increased, the functional capabilities of the limb are improved, manifesting in the results of functional tests improving by one point during tests with a five-point rating scale and by 10 points during tests with numerical results, and when the electrogenesis of the flexor muscles of the elbow joint m. biceps brachii and m. brachialis is reduced by more than 100 mcV according to EMG data, the result is considered positive.

EFFECT: method ensures the prediction of the results of selective neurotomy of the motor branches of the musculocutaneous nerve in children with cerebral palsy due to the modelled functional result of selective neurotomy of the musculocutaneous nerve, allowing for clear differentiation of the tonic and fixed contractures of the elbow joint, providing the possibility of accurately determining the primary cause of limb dysfunction and, accordingly, selecting the most effective method for further surgical treatment.
